Associate Medical Director – PACE program
The PACE is program for all inclusive care of the elderly and our goal is to keep seniors 55+ independent and living in the community for as long as possible. The participants are approved by the state as nursing level of care and we approach their healthcare with an interdisciplinary team approach (NP PCP, social work, RN case manager, home health, PT/OT, engagement coordinator for socialization/activities, dietician etc.).
The Associate Medical Director would help oversee the clinical aspects of patient care and work with the Medical Director to identify trends in patient care and develop quality improvement plans and processes.
Some specific responsibilities include:
-Train and oversee NPs to ensure quality value based care (act as an attending and preceptor: review notes, provide education and feedback)
-Assist NPs with same day acute visits if needed in the clinic (urgent care to avoid ER utilization)
-Occasionally cover inbox or patient appts if PCP is out
-Pre-enrollment interviews with prospective patients to ensure they meet criteria and we can safely provide continuity of care
-Help follow inpatient hospital course and SNF care with RN case mgrs.
-Observe clinic flow, identify areas of improvement and work with clinic manager, RN supervisor to enact change
-Attend daily morning clinic huddle to hear about overnight events and help prioritize patients that need follow up
-The clinic is open M-F 8-430, possibility of 1 day remote
-Every 6-8wks take back up call (by phone) to assist NP if any questions arise (typical volume 0-3 calls/night)
Requirements:
Board Certified in Family Medicine, Internal Medicine, or Geriatrics
Licensed in California
