Outpatient practice Monday through Friday.
Practice has 7 Family Medicine physician and 3 Nurse practitioners
Office Call Rotation: Minimal call by phone

Located close to South Bend and 1.5 hours from Chicago; community of 50,000 with a county service area of 200,000.
