Primary Care Physician – PACE program
The PACE is program for all inclusive care of the elderly and our goal is to keep seniors 55+ independent and living in the community for as long as possible. The participants are approved by the state as nursing level of care and we approach their healthcare with an interdisciplinary team approach (IDT) (NP PCP, social work, RN case manager, home health, PT/OT, engagement coordinator for socialization/activities, dietician etc.).
The Primary Care Physician performs in-person comprehensive assessments and coordinates with the IDT to develop a care plan for each participant. At the direction of the Medical Director, the Primary Care Physician oversees all care of participants in nursing homes and hospitals including admits and discharges for acute, short-term, and long-term placement and the use of medical specialists to ensure effective care coordination. The Primary Care Physician also provides direction and support to Nurse Practitioners and other members of the IDT.

Requirements:
Board Certified in Family Medicine, Internal Medicine, or Geriatrics
Licensed in California
