Responsibilities:
- Conduct behavioral health assessments and provide psychotherapy interventions
- Develop individualized treatment plans
- Collaborate with physicians, nursing staff, and facility care teams
- Maintain timely and accurate clinical documentation
- Support emotional wellness and quality of life for older adult patients
Qualifications:
- Active Indiana independent practice license as LPC, LMHC, or LMFT
- Strong assessment, diagnostic, and counseling skills
- Ability to work independently and manage schedule efficiently
