Compensation & Benefits:
- $232K–$264K+ base FFS compensation with uncapped earning potential

Responsibilities:
- Provide comprehensive patient assessments and ongoing care management
- Lead quality initiatives and collaborate with facility leadership
- Coordinate care with interdisciplinary teams
- Mentor clinicians and support operational excellence
- Maintain timely, compliant documentation
Qualifications:
- MD/DO with New York license and DEA (or ability to obtain)
- IM, FM, or Geriatrics background preferred
- Post-acute, inpatient, ED, or clinic experience welcomed
