Compensation & Benefits:
- $352,751 – $390,021+ with uncapped earning potential

Responsibilities:
- Comprehensive patient assessments and ongoing care management
- Manage acute and chronic conditions in post-acute settings
- Lead quality initiatives and collaborate with facility leadership
- Coordinate care with nursing staff and interdisciplinary teams
- Educate patients and families
- Maintain timely documentation and quality standards
Qualifications:
- BC/BE in Internal Medicine, Family Medicine, or Geriatrics
- North Carolina license and DEA (or ability to obtain)
- Post-acute, inpatient, ED, or clinic experience welcomed
