Compensation & Benefits:
- $221K–$256K+ FFS compensation with uncapped earning potential

Responsibilities:
- Perform patient assessments and ongoing care management
- Lead quality initiatives and collaborate with facility leadership
- Coordinate care with nursing staff and interdisciplinary teams
- Support operational excellence and patient outcomes
- Maintain accurate, timely documentation
Qualifications:
- Board Certified in Internal Medicine, Family Medicine, or Geriatrics
- South Carolina license and DEA (or ability to obtain)
- Post-acute, inpatient, ED, or clinic experience welcomed
