Responsibilities:
- Manage patient care and complex conditions in post-acute setting
- Lead quality initiatives and collaborate with facility leadership
- Coordinate care with interdisciplinary teams
- Maintain timely, compliant documentation
Qualifications:
- BC/BE in Internal Medicine, Family Medicine, or Geriatrics
- North Carolina license + DEA (or ability to obtain)
- Post-acute or related experience preferred
