Primary Care NP/PA (FP, IM, Geriatrics) – Greater Hartford Area, CT
- Full-time 5-day/week in skilled nursing/post-acute care
- Monday–Friday round-and-go schedule
- Strong autonomy with clinical and administrative support
- Dedicated after-hours/night-time backup call
- Post acute care experience required
