Primary Care Physician (IM / FM / Geriatrics) – Grand Rapids Area, MI

Compensation & Benefits:
- $220K–$300K+ FFS compensation with uncapped earning potential

Responsibilities:
- Perform patient assessments, H&Ps, and care planning
- Manage acute and chronic conditions in post-acute settings
- Coordinate care with nursing staff and interdisciplinary teams
- Educate patients and families
- Maintain accurate and timely documentation
Qualifications:
- MD/DO with Michigan license and DEA (or ability to obtain)
- IM, FM, or Geriatrics background preferred
- Post-acute, inpatient, ED, or clinic experience welcomed
