Psychiatric Mental Health Nurse Practitioner

Position Overview:
We are looking for an experienced Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team in delivering psychiatric care to our senior participants.
Key Responsibilities:
- Provide psychiatric consultations, evaluations, counseling, and treatment in partnership with physicians and the IDT.
- Conduct comprehensive psychiatric assessments, either in-person or via telehealth, and contribute to individualized care plans for each participant.
- Assist in discharge planning for participants transitioning from acute or long-term care settings.
- Collaborate with physicians to oversee participant care in nursing homes, conducting regular visits as required.
- Act as a consultant to the IDT, communicating changes in participant status and assisting in care coordination.
- Refer participants to specialized psychiatric services when needed.
Qualifications:
- Active state licensure as a PMHNP.
- Valid DEA and CDS certifications.
- At least 2 years of experience as a PMHNP, with geriatric experience preferred.
Compensation and Benefits:
- Competitive salary range: $155,000 - $185,000 + Bonus + Equity
