RN Care Manager 

The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.

Required qualifications:

• Bachelor's Degree
• Upon hire: California Registered Nurse License 
• 2 years of Acute hospital experience
• 1 year of supervisory experience.
• Experience in Case Management (Care Coordination or Utilization Management)

Skills and Certifications:

CA RN License
 2 years of Acute hospital experience
 1 year of supervisory experience.
 Bachelor's Degree

Compensation:

• Base Salary - USD $106,995 to $172,764
